Have you or your children already used Duality Healthcare (or Duality Health)? When you try and register for membership the system may recognise that you already exist (via your e-mail) and the following message will appear:
“This account is already registered on our system. Please click here to reset your password or contact us for further support”
Click “click here” to reset your password and gain access to the Patient Portal. Previous users of Duality Health(care) are automatically registered with the Patient Portal.

- For Individual Membership select: Silver – Individual Membership
- Proceed with your application
- For Family Membership select: Silver – Family Membership – Lead Parent
- You will be taken through your registration as the Lead Parent
- Once complete you will be sent an e-mail detailing how to e-mail us for the registration for your remaining family members
- You will then be sent an email with codes to register the other members of your household
